Here’s how to get started with Counseling:

  • Schedule your first appointment:
    • Please call my confidential voice mail at 928-445-0055 to schedule an appointment.
      My office is located in Prescott Valley AZ.
      I also offer Online Therapy via computer or smartphone. 
  • Complete the initial paperwork:
         a.  Read the Notice of Privacy Practices.
         b.  Read & sign the Informed Consent: Informed Consent – In Office and/or Informed Consent – Online
         c.  Complete the Intake Questionnaire 
  • Pay for services at the beginning of each appointment.
    • In-Office appointments: Payment can be done with cash or a check in the office, or via US Mail.
    • Online appointments: Payment can be done via the telemed platform, or via US Mail.
